Of all the common nutrient deficiencies, vitamin B12 has one of the most deceptive timelines. Unlike vitamin C, which the body depletes within weeks without dietary intake, your liver stores roughly two to five years worth of B12. This means someone can stop absorbing or consuming adequate B12 today and feel completely normal for years — which is precisely what makes it dangerous rather than reassuring.
The four-stage progression worth knowing
B12 deficiency doesn't appear suddenly; it follows a recognizable pattern, and knowing where you might be on it matters for how urgent testing is:
- Silent depletion. Stores are dropping but still sufficient — this stage can last years, and it's where many long-term vegans, vegetarians, and metformin users sit without realizing it.
- Vague symptoms. Persistent fatigue, weakness, paler skin, feeling winded climbing stairs, low mood. Easily attributed to stress, poor sleep, or simply aging.
- More specific signs. A sore, smooth, beefy-red tongue (glossitis), mouth ulcers, and tingling or pins-and-needles sensations in the hands and feet.
- Neurological involvement. Numbness, balance and coordination problems, memory difficulties and confusion. At this stage, treatment becomes urgent — some neurological changes can become permanent if the deficiency continues untreated.
A critical wrinkle: the neurological symptoms in stage 3 and 4 can appear before any anemia shows up on a routine blood count. Feeling persistently "off" with a normal hemoglobin result does not rule out B12 deficiency.
Who actually needs to pay close attention
B12 exists naturally only in animal-derived foods, and absorbing it from food requires adequate stomach acid and a protein called intrinsic factor. Nearly everyone who becomes deficient has a problem with one of those two things — supply or absorption:
- Vegans and vegetarians — no animal foods means no natural dietary B12 source. Fortified foods or supplementation aren't optional long-term choices; they become essential.

Where B12 deficiency gets complicated is recognition; treatment itself is straightforward. A serum B12 blood test is the standard first check, sometimes followed by methylmalonic acid or homocysteine testing when results fall in an ambiguous middle range. Treatment — high-dose oral B12 or injections — is effective and well-tolerated, and the oral form works even for many people whose underlying problem is food-bound absorption specifically.

The B12-folate interaction worth knowing
One clinically important detail often missed: folate (vitamin B9) and B12 deficiency can mask each other on standard blood tests. Both cause a similar type of anemia, and high-dose folic acid can normalize the blood-count abnormality that would otherwise be a clue pointing toward B12 deficiency — while the underlying nerve damage from unaddressed B12 deficiency continues progressing untreated. This is one reason clinicians often check both together rather than assuming a normal-looking blood count with adequate folate rules out a B12 problem.
Why plant-based eaters can't simply "eat more greens" to fix this
A common and reasonable-sounding assumption is that a sufficiently varied plant-based diet can cover B12 the way it covers most other nutrients. It genuinely cannot — B12 is produced by bacteria and exists naturally only in animal-derived foods (meat, dairy, eggs) or foods fortified specifically to include it. This is different from nutrients like iron or zinc, which have meaningful (if less efficiently absorbed) plant sources.
